Mimi Choate, MD, MPH

Dr. Mimi Choate earned her dual MD-MPH degree from Oregon Health & Science University (OHSU) and completed her family medicine residency training at Poudre Valley Hospital in Fort Collins, Colorado. She has been proud to call the Rogue Valley home since 2012 and is thrilled to bring her comprehensive, compassionate approach to Valley Family Practice in Medford.

Specializing in multi-generational medicine, Dr. Choate provides inclusive care for men, women, and children at every stage of life. From newborn milestones, specialty women’s health, and patient-centered chronic disease management to specialized, stigma-free recovery support through addiction medicine, she focuses on treating the whole person with empathy and respect.
